【IMPORTANT NOTICE】 Data Transfer from JC LevelMind to TO_MO
Dear Service Users,
Thank you for your long-standing support of JC LevelMind (hereinafter referred to as "LevelMind").
Please be informed that the "LevelMind@JC" and "LevelMind@JC (Staff ver.)" applications, formerly managed and operated by The Boys' & Girls' Clubs Association of Hong Kong, along with the relevant service systems containing LevelMind members’ records across various operating agencies, will officially merge with "TO_MO by 21C@JC" application and system (hereinafter referred to as "TO_MO") on January 27, 2026. TO_MO is managed and operated by The Hong Kong Federation of Youth Groups.
To maintain your seamless experience with LevelMind’s online system services, your personal data will be migrated to TO_MO on the date of migration in accordance with the merger agreement.
【Details of Data Migration】
Data Recipient: TO_MO by 21C@JC Application
Purpose of Data Usage: Your personal data will be utilized solely by your affiliated LevelMind hub and Integrated Children and Youth Services Centre to continue providing relevant services, managing your account, and processing your inquiries. These purposes remain directly related to the original purposes for which the data was originally collected.
Data Protection: The TO_MO by 21C@JC application pledges to comply with the Personal Data (Privacy) Ordinance (Cap. 486) of the Laws of the Hong Kong Special Administrative Region, to adopt reasonable precautionary measures and use all reasonably practicable efforts to ensure your personal data is properly safeguarded following the migration to TO_MO.
The Hong Kong Jockey Club Charities Trust, which created and funded the LevelMind project, does not have the authority to access any personal data contained within the application.
The Hong Kong Federation of Youth Groups, which manages and operates the TO_MO application, hereby declares that, except for necessary technical support required to maintain the operation of TO_MO or as required by legal mandates, it has no authority to access any personal service data belonging to the LevelMind project. This includes, but is not limited to, member accounts, activity participation records, questionnaire data, case records, and relevant service files. This is to ensure that the privacy of LevelMind members receives independent protection.
However, if a member registers for or participates in events hosted by other agencies (including other collaborating partners of LevelMind or other non-LevelMind service agencies) via TO_MO, it shall be deemed as your express consent and authorization for TO_MO to disclose the personal data (limited to name, contact number, email address, and the particular activity records) to the hosting agency for communication and administrative purposes. In such cases, the processing of relevant data will be subject to the privacy policy of the said hosting agency. Service users should take note of this arrangement.
【Important statement on Scope of Transfer and Eligible Subjects】
Pursuant to the personal data migration plan, LevelMind will migrate the data of service users within the following scope to the TO_MO system on January 27, 2026:
Eligible Subjects: Individuals registered as "Members" or "Temporary Members" of LevelMind who meet at least one of the following conditions on or after November 1, 2024:
- Service user who has logged into the LevelMind@JC membership system; or
- Has successfully registered as a LevelMind member; or
- Has completed the "2M Questionnaire"
Categories of data to be transferred: The scope of transfer includes personal identifiers, contact information, account records, and relevant questionnaire data registered by the above-mentioned LevelMind service user.
Note: If you do not meet any of the above conditions, your personal data will not be migrated to TO_MO and will be handled by the operating agency of your respective LevelMind hub in accordance with their policies.
【Opt-out from Data Migration】
If you have any inquiries regarding the data transfer or wish to opt-out from migration of your personal data to TO_MO, please notify the TO_MO Team through written notification on or before January 18, 2026:

【Statement of Important Consequences】
Please Note: If you choose to opt-out from data migration, we will be unable to retain your LevelMind account and data (including service records) within the TO_MO system, hence online system services will be discontinued. This implies your LevelMind system account and data will be officially terminated and deleted on January 27, 2026.
【Right to access and correction of personal data】
Pursuant to the Personal Data (Privacy) Ordinance, you have the right to request access and correction of your personal data held by TO_MO. To exercise these rights, please contact us via the following methods:
